非機能性悪性膀胱傍神経節腫の1例

A CASE OF NON-FUNCTIONING MALIGNANT PARAGANGLIOMA OF THE URINARY BLADDER

　無症候性肉眼的血尿を主訴に経尿道的切除にて診断された54歳，男性の非機能性悪性膀胱傍神経腫の1例を報告する。術中も含め高血圧は全く認めず，血中，尿中カテコラミンもすべて正常であったが，膀胱部分切除時施行した後腹膜リンパ節郭清にて右閉鎖節に1個転移を認めた。非機能性褐色細胞腫は稀であり，悪性かつ異所性（傍神経節腫，特に膀胱原発）はさらに稀であると考えられる。

A 54-year-old male, who had been suffering from gross hematuria, was referred to us after transurethral resection of bladder tumor with suspicion of malignant paraganglioma. He had never experienced typical symptoms of paraganglioma of the urinary bladder, such as hypertension and attack on micturition. Plasma and urinary catecholamines, examined twice after tansurethral resection, were within normal limit. CT scan, which was performed before transurethral resection, revealed only thickening of the left bladder wall. ¹³¹I-metaiodobenzylguanidine scintigraphy failed to reveal abnormal uptake.
