対側腎盂・尿管に転移した腎癌

URETERAL AND RENAL PELVIC METASTASIS FROM CONTRALATERAL RENAL CELL CARCINOMA

　71歳，女性。無症候性血尿を主訴に受診。左腎腫瘍に対し左腎摘出術施行。病理学的に腎細胞癌，stage IV Aであった。約10ヵ月後に血尿，さらに右尿管閉塞による無尿となり，右尿管皮膚瘻術施行。一部切除した右尿管壁，および腎盂より排出された組織片内に左腎癌と同じ腫瘍組織が認められた。周囲からの直接浸潤なく，腎癌の対側腎盂尿管転移と診断した。

A case of a 71-year-old female patient with a metastatic renal cell carcinoma of rt. ureter and renal pelvis from It. kidney was reported. In July, 1986 It. nephrectomy was performed. In May, 1987 she was readmitted to our hospital with chief complaints of hematuria and hypertension. After a month, she had episodes of anuria and rt. ureterocutaneostomy was done. The 10 × 20 mm polypoid body in the urine was encountered, which was histologically suggested from renal pelvis and finally diagnosed as clear cell carcinoma.
