副睾丸嚢腫捻転症

A CASE OF TORSION OF EPIDIDYMAL CYST

　患者は65歳男性。40年前頃より左陰嚢内の腫瘤に気づいていたが放置。4日前より左陰嚢部の疼痛，腫脹に気づき，1987年3月23日当科受診。左睾丸回転症を疑い手術を施行した。左副睾丸頭部に単発性で暗赤色の腫瘤を認め，茎部にて360度内旋していた。副睾丸嚢腫捻転症と診断し，茎部にて嚢腫の摘除を行った。組織学的に嚢胞壁は副睾丸由来と考えられた。

A 65 year-old patient was admitted with the chief complaint of a swelling and severe pain in the left scrotum during the preceding 4 days. Physical examination revealed a tender mass of about small hen-egg size at the left epididymal region. Surgical exploration was performed and the testis was unaffected, but a freely movable cyst twisted 360 degrees around the thin base was attached to the head of epididymis. The mass was then removed. Histological study showed the cyst to be epididymal duct in origin with diffuse hemorrhage in the wall.
