尿細胞診の診断的意義

STUDY ON URINARY CYTOLOGY OF BLADDER TUMOR

　最近4年間の膀胱腫瘍患者203例（男153例，女50例），および良性疾患患者358例についての尿細胞診の成績をまとめた．Class Ⅲ以上を陽性とすると敏感度は69.5％，特異度は95.5％であった．また腫瘍の数，異型度，深達度が増すほど陽性率は上がること，初発例の方が再発例より有意に陽性率が高いこと等を確認した．細胞収集等の問題点を解決すれば尿細胞診をマススクリーニングに広く応用できると思われる．

The efficacy of urinary cytology in the detection of bladder carcinoma was estimated. The study involved 203 bladder tumors (153 men and 50 women) and 358 non-malignant diseases in Tokyo Medical College Hospital from May 1985 to April 1989. The urine samples were stained and examined blindly by the same pathologist and cytoscreener according to the method of Papanicolaou. Exforiated cells were classified as class Ⅰ~Ⅴ and class Ⅲ~Ⅴ were regarded as cytologically positive. Of 203 patients with bladder carcinoma 141 showed positive cytological results while only 16 positive cases were recognized in 358 non-malignant diseases.
