　10年来人工透析を受けていた33歳の男性が透析中突然に陰茎の勃起と疼痛を自覚し，それが持続するため翌日当科で受診した。持続勃起症の診断のもと，ウロキナーゼ点滴静注ヘパリン加生食による海綿体洗浄を施行，2日目より軽快，14日目には形態的に正常となった。6ヵ月後の現在，以前ほどではないが勃起し射精も可能である。透析後の持続勃起症としては本邦2例目であり，原因としてのヘパリンの関与に言及した。

A 33 year-old man who had been on hemodialysis for 10 years had a painful erection. He was hospitalized on the next day with priapism. Drip infusion of urokinase and irrigation of the corpora by saline with heparin were continued for 14 days. Within 48 hours pain and tenderness disappeared. On the 14 th day of therapy the penis was normal in appearance and consistence. Six months later he had normal sexual intercourse. This is the second case of hemodialysis associated priapism in Japanese literatures and the possible causative role of heparin was discussed.
