生検にて診断した早期腎細胞癌

EARLY RENAL CELL CARCINOMA DIAGNOSED BY ULTRASONICALLY GUIDED RENAL BIOPSY AT OPERATION

　46歳，女性。乳癌術後の定期検査の際，偶然CTにて左腎腫瘤を指摘され，当科に紹介された。CT，エコーなどで腎腫瘍が疑われたが，確診は得られず，術中の生検にて腎細胞癌の診断を得たため，左腎摘除術を施行した。腎の微小病変の診断ば，種々の検査を施行しても困難な場合がある。腎腫瘍を否定しきれない場合には，生検がひとつの手段であるが，より安全に確実に行うためには，開腹手術によるエコーガイド下の生検が良いと思われた。

We reported a case of early renal cell carcinoma incidentally detected by abdominal CT. In this case further examination including IVP, CT, echography and angiography were unable to differentiate benign mass from malignant mass. Therefore we performed an exploratory laparotomy followed by ultrasonically guided renal biopsy. The diagnosis was renal cell carcinoma and left nephrectomy was performed.
