小さい腎オンコサイトーマ

SMALL RENAL ONCOCYTOMA

　53歳，男性．腹部CTで偶然に右腎腫瘤を指摘され当科紹介となる．超音波検査，腎動脈造影等を行ったが確診が得られず，開放生検を行った．術中迅速病理では腎細胞癌の疑いとの診断で，右腎摘除術を施行した．摘出標本では，腫瘍は10×15mm大と小さく，被膜を有する暗褐色調の腫瘍であり，光顕，電顕的にオンコサイトのみからなる腎オンコサイトーマと診断した．微小腎オンコサイトーマの術前診断の困難さを感じさせる1例を報告した．

A 53-year-old man was referred to our clinic who had a small mass on the righ kidney by plain computed tomography. Ultrasonography and enhanced computed tomography did not help to establish the diagnosis and renal angiography showed no evidence of abnormality. Therefore operative biopsy was performed on suspicion of malignancy and a total nephrectomy was performed under the diagnosis of renal cell carcinoma.
