膀胱偽肉腫型線維粘液様腫瘍の1例

PSEUDOSARCOMATOUS FIBROMYXOID TUMOR OF URINARY BLADDER : A CASE REPORT

　37歳，女性。排尿終末時痛，左下腹部痛にて受診。1回目の生検術で脂肪肉腫が疑われたが，2回目の生検術にて，偽肉腫型線維粘液様腫瘍と診断され，膀胱部分切除術が施行された。紡錘細胞，粘液様間質が見られる膀胱腫瘍は稀であり，しばしば良，悪性の鑑別が困難である。電顕や免疫組織学的な検索も行い，悪性と診断し拡大手術を行わぬことが重要である。

A 37-year-old woman was admitted with a one month history of terminal miction pain and left lower abdominal pain. Physical examination and laboratory findings were essentially unremarkable. Cystoscopy, CT and MRI revealed a 3 × 3 × 3. 5 cm tumor located at the left anterior wall of the urinary bladder. Transurethral biopsy was done. The specimens were initially diagnosed as myxoid sarcoma, most likely liposarcoma. However TUR was repeated and showed a pseudosarcomatous fi-bromyxoid lesion with a typical spindle cells.
