前立腺全摘除術における貯血式自己血輸血

AUTOLOGOUS BLOOD TRANSFUSION IN RADICAL PROSTATECTOMY

　前立腺全摘除術を施行した前立腺癌患者16名に対してエリスロポエチンを併用して術前に800mlの自己血を貯血し，術中・術後に使用した。貯血によるヘモグロビン値の低下は平均1.3g／dlであり，エリスロポエチンを併用することによりヘモグロビン値の低下を防ぐことができた。2例で同種血輸血を必要としたが，14例（87.5％）では自己血輸血のみで術後のヘモグロビン値は10g／dl以上に保たれ，術後血圧も安定していた。前立腺全摘除術の多くは，同種血輸血を用いずに自己血輸血を行うことで安全に手術を施行することができると考えられた。

Sixteen patients with prostate cancer underwent radical prostatectomy with autologous blood transfusion. Before operation, 800 ml blood was collected in each patient with intravenous erythropoietin administration. The aver-age decrease of hemoglobin value was 1.3 g/dl before the operation. Only 2 patients required additional transfu-sion of congener blood. We conclude that the autologous blood transfusion is a safe and useful method in radical prostatectomy.
