　一期的形成術を行った近位型尿道下裂28例を膀胱瘻群（経皮的膀胱瘻＋尿道留置）と尿道留置群（尿道留置のみ）に分けて尿のドレナージ法につき検討した。1）膀胱刺激症状は膀胱瘻群では全例にみられ，強度のものが多かった。2）少量の尿道留置カテーテル脇からの排尿は新尿道の閉塞がない限り問題にならないと思われたが，カテーテルの閉塞による大量のカテーテル脇からの排尿は新尿道を損傷する恐れがある。3）初回手術成功率は膀胱瘻群54.4％，尿道留置群64.7％であった。以上よりカテーテルの閉塞に注意をすれば近位型下裂でも尿道留置のみで十分と考えられた。

Single stage repairs have been developed in severe types of hypospadias since last 5 years in our clinic. Management of urinary drainage is one of the most important factors for success. Though we favored the suprapubic cystostomy with intraurethral stent in the first series (11 cases), the bladder irritability and its affiliated voiding around the catheter were remarkable. Then, in the second series (17 cases), indwelling urethral catheter alone was tried for urinary drainage. There were less bladder irritation and little voiding around the catheter without catheter obstruction in the second series.
