腹腔鏡下前立腺全摘除術

Laparoscopic Radical Prostatectomy

　前立腺全摘除術では，切除断端における癌の完全切除と，尿道括約筋の損傷を抑えて尿禁制を保つことの両方が要求される．しかし前立腺は膀胱と尿道とに連続する臓器で，骨盤の最深部に位置する．この解剖学的特徴が前立腺全摘除術を困難なものにしている．腹腔鏡はその拡大視野で，前立腺全摘除術を深く狭い場所で行うのを助けるが，このような手術を安全かつ短時間で確実に行うには，正確な解剖の理解，的確な器具の選択，さらにそれぞれの手技の十分なトレーニングが要求される．腹腔鏡下前立腺全摘除術のそれぞれのステップにおけるコツとピットフォールについて述べる．

Radical prostatectomy requires both cancer-negative margin and urinary continence without major injury of the urethral sphincter. However, the prostate is the organ jointed to the bladder and urethra in the deepestpo-sition in the pelvis. This anatomical characteristics makes radical prostatectomy difficult. While laparoscopy might help to perform radical prostatectomy in the deepest narrow space under magnified view, understanding of the exact anatomy, selection of the proper instruments and enough training of each procedure are required to do it safely and promptly.
