背部圧痛点への局所麻酔による尿路結石疝痛の治療の試み

NEEDLE PUNCTURE AND LOCAL ANESTHETIC BLOCK OF TENDER SPOTS FOR URETERO-RENAL COLIC

　尿路結石疝痛を訴える48名，延べ54件の患者に対し，背部圧痛点（患側の第3腰椎側方で，腎兪・志室と称する2経穴と場所が一致）への穿刺と局麻を行った。局麻剤として，リドカイン単独を11件（1群）に，リドカイン＋デキサメタゾンを11件（2群）に，リドカイン＋アドレナリンを残る32件（3群）に用いた。全例に速効性の鎮痛を得たが，鎮痛の程度は治療前の圧痛の強さに有意に相関し，使用した薬剤とは無関係であった。ただし24時間以内の疼痛再発は，再受診した45件中の41.3％（1群；60％，2群；40％，3群；34.6％）に達したが，2件を除き治療前の疼痛レベル以下であった。

　

We studied the effect of xylocaine injection to the tender spots for ureterorenal colic. Fourty-eight urolithiasis patients with 54 colicky attacks were treated with needle puncture and local anesthetic block of dorsal tender spots which were coincident with two separate acupancture spots, namely "Jinyu" and "Shishitsu" located at the lateral position of the third lumbar vertebrae. Three different groups were studied.
