精索に発生した悪性線維性組織球腫

Malignant fibrous histiocytoma of the spermatic cord

　症例は73歳，男性。左側鼠径部から陰囊基部にかけて多発する腫瘤を主訴に入院した。12か月前には，左側高位精巣摘除術を施行され，悪性精索線維性組織球腫と診断された既往を有していた。左側鼠径部から陰囊基部にかけて多発性腫瘤を認め，これらを周囲脂肪組織と一塊に摘出した。手術標本は前回と同様の組織診で，被膜に覆われた状態で摘出が可能であった。補助療法は施行しなかったが，術後1年を経過した現在，再発を認めていない。悪性精索線維性組織球腫は稀な疾患であり，本症例はわれわれが調べ得た限りでは本邦20例目にあたる。

　A 73-year-old man was hospitalized because of the left scrotal and inguinal tumors. He had an episode of the left orchiectomy 12 months before and histological diagnosis was malignant fibrous histiocytoma of the spermatic cord. At admission, multiple tumors from left inguen to the left basal scrotum was observed, and complete en bloc excision of those tumors was performed. Pathologic diagnosis of tumors was the same as previous surgery. The neoadjuvant therapy was not done, yet recurrence is not recognized over one year. Malignant fibrous histiocytoma is a rare disease, and this case seemed to be the 20th case reported in Japanese literatures to our knowledge.
