腎に浸潤した膵癌の1例

A PANCREAS CANCER WITH INVASION TO THE KIDNEY: A CASE REPORT

　症例は65歳の男性。無症候性血尿を主訴に1984年9月初診。膀胱鏡にて左腎性血尿，IVPで左無機能腎，CTスキャンで左腎上極から後腹膜腔に連続する腫瘤を認めた。手術では左腎上極を中心に膵，脾への浸潤が強く摘出不能，生検の結果膵尾部原発の腺癌で左腎への浸潤と診断された。化学療法を行つたが効なく3カ月後死亡。若干の文献的考察とともに報告する。

A 65-year-old man was admitted complaining of asymptomatic grosshematuria. Cystoscopic exami-nation showed left renal hematuria. Excretory urogram revealed left nonfunctioning kidney. CT scan demonstrated a mass in upper pole of the left kidney, spreading to the retroperitoneal space. Surgical removal of the mass was attempted but was not successful because the mass was highly invasive to the pancreas and the spleen. Histological finding of the biopsied specimen showed adenocarcinoma of the pancreas. Chemotherapy was done but he died 3 months later. Secondary renal tumor was briefly discussed.
