自然破裂を来した腎血管筋脂肪腫の1例

A CASE OF SPONTANEOUS RUPTURE OF ANGIOMYOLIPOMA

　38歳，女性で右背部痛を主訴に来院，血液検査で高度の貧血とLDHの上昇を認めた。超音波検査，CTスキャンおよび血管造影により腎血管筋脂肪腫の自然破裂が疑われ，検査と同時に腎動脈塞栓術を施行。止血を確実にするため塞栓物を少し多く使用，硬塞範囲がやや広範となつたため右腎切除術を余儀なくされた。本邦の本腫瘍の16.5％に自然破裂が認められ，その84％が腎切除術を施行されており，今後，腎動脈塞栓術による治療の検討が期待される。

A case of spontaneous rupture of angiomyolipoma in a 38-year-old woman with the chief complaints of right flank pain was reported. Physical examinations revealed a large right renal mass. Laboratory studies disclosed a decrease of Hb to 7.6g/dl and Ht to 23.4%, and elevated LDH to 505 U/l. Ultra-sonography, CT scan and angiography showed characteristics of angiomyolipoma complicated with spon-taneous rupture.
