馬蹄腎に合併した腎血管筋脂肪腫破裂

Rupture of angiomyolipoma in a horseshoe kidney：A case report

　症例は50歳，女性。左側腹部痛を主訴に近医を受診した。腹部CTにて馬蹄腎，左腎動脈瘤および左腎血管筋脂肪腫破裂と診断された。貧血の進行に対して輸血と経皮的左腎動脈瘤塞栓術が施行された。手術目的で当院に転院し，経腹的左腎摘除および峡部離断術を施行した。組織診断で術前診断が確認された。術後腎機能低下はみられず，術後経過も順調で入院後20日目に退院した。現在まで再発はみられない。

　A 50-year-old woman presented with the chief complaint of left flank pain. CT scan and bilateral pyelography and MRI revealed left renal aneurysm and ruputure of left angiomyolipoma in a horseshoe kidney. After embolization of the left renal aneurysm and blood transfusion for severe anemia, heminephrectomy and resection of the renal isthmus was performed, and histological examination confirmed the preoperative diagnosis. Renal dysfunction did not occur and she was discharged uneventfully. No recurrence has been seen to the present.（Rinsho Hinyokika 61：553-555, 2007）
