腎自然破裂を来した腎細胞癌

A Case of Spontaneous Rupture of Renal Cell Carci-noma

　症例は64歳，男性。仕事帰りのバスの中で突然右側腹部痛が出現した。疼痛が徐々に増強し，救急車にて当院内科を受診した。腹部CT上，内部不均一な造影のある直径4cmの腫瘤と，その周囲に被膜外へ進展する血腫様の像を認めた。腎細胞癌による腎自然破裂が疑われたため，2週間後，胸腹斜切開にて根治的右腎摘除術を施行した。病理組織は腎細胞癌であり，術後6か月が過ぎた現在も転移，再発は認めていない。

A 64-year-old man was admitted to our hospital on an emergency because of a sudden right flank pain. There was no evidence of recent trauma. Abdominal computed tomog-raphy revealed a 4cm right renal tumor enhanced by contrast medium with perirenal hematoma, but showed no active bleeding. Angiography showed hypovascularity of the tumor. Suspecting spontaneous rupture of renal cell carcinoma, we performed radical nephrectomy.
