一期的全尿道形成術後の尿道皮膚瘻に対する治療成績

REPAIR OF URETHROCUTANEOUS FISTULAS AFTER ONE STAGE URETHROPLASTY

　過去5年間に近位型をも含め，69例の尿道下裂症例に対して一期的全尿道形成術（onestage urethroplasty with parameatal flap）を施行した。52例，75.4％に初回手術で成功をおさめたが，17例，24.6％は合併症のため，再手術を要した。要再手術例17例中12例が持続性の瘻孔形成例であり，これら症例につき検討を加えた。瘻孔閉鎖術の方法および成績を述べた。12例中10例に瘻孔閉鎖術を行い，治癒せしめた。他の2例は手術待機中である。

From 1979 to 1984, without regard to the type of hypospadias, 69 patients underwent one stage ure-throplasty with parameatal flap (OUPF). In 52 cases (75.4%), the procedure was primarily successful, while in the other 17 cases additional procedure was required for variety of complications. Permanent urethrocutaneous fistula was the most often encountered complication and seen in 12 cases. Of these, closure of fistula was primarily successful in 10 cases with 2 patients still on the waiting list for fistulec-tomy. Some discussion was made as to the cause, prevention and repair of fistula.
