進行性膀胱腫瘍，前立腺腫瘍に対する選択的動注療法の経験

CLINICAL TRIALS OF THE SELECTIVE INTRA-ARTERIAL INFUSION OF CDDP AND ADM TO THE ADVANCED CARCINOMA OF THE URINARY BLADDER AND PROSTATE

緒言

　膀胱癌に対する根治的治療としては，膀胱全摘および尿路変更術が一般に行われているが，進行癌や高齢による一般状態不良例あるいは重篤な合併症のある例で根治手術不能例に遭遇することは稀ではない。これらの症例に対しては抗腫瘍剤化学療法や放射線療法あるいは免疫療法に頼らざるをえない。しかし，膀胱腫瘍に対する抗腫瘍剤の化学療法に関しては，薬剤やその投与方法が進歩したとはいえ，未だその効果は確立されたわけではない。

　私たちは，膀胱癌の全摘不能例とホルモン抵抗性前立腺癌に対し，cis-diaminedichlorideplatinum（以下CDDPと略す）とDoxorubicin hydrochloride（以下ADMと略す）を下臀動脈より内腸骨動脈に留置したカテーテルより注入し良好な結果を得たので症例を紹介し，若干の文献的考察を加えた。

For the purpose of the treatment of the advanced prostate and bladder carcinoma, inoperable five patients were treated by selective intra-arterial infusion of adriamycin and diamminedichloride platinum. The catheter was inserted from the inferior gluteal artery into the internal iliac artery. The basic dosage of ADDP was 10mg/day twice a week for 5 weeks and of the other agent was 10mg/day once a week for 5 weeks. In one case, prostatic transitional cell carcinoma invasive to the bladder with right hydro-nephrosis, the tumor in the bladder disappeared endoscopically and reduced endoscopically and phisically in the prostatic urethra.
