後腹膜原発の悪性線維性組織球腫の1例

A CASE OF RETROPERITONEAL MALIGNANT FIBROUS HISTIOCYTOMA

緒言

　組織球に由来する悪性腫瘍のうちで，悪性線維性組織球腫（malignant fibrous histiocytoma以下MFH）は，主として四肢に多く発生し，後腹膜に発生する例は全体の10数％に過ぎないとされている。今回，われわれは，42歳男性で，右季肋部痛を主訴として来院した患者で，腎周囲の軟部組織より発生したMFHの1例を経験したので，若干の文献的考察を加えて報告する。

A 42-year-old male was presented with vague right upper abdominal pain. Physical examination revealed a large mass with relatively smooth surface in the right upper quadrant. The pertinent positive laboratory data included increased ESR, hyper-α₂-globulinemia and hyperfibrinogenemia. CRP was positive as well. Adrenocortical hyper-activity was not elicited. Abdominal CT scan demonstrated a huge retro-peritoneal mass locating just above the right kidney. Renal angiogram was compatible with the neovas-cularity of the tumor feeded by the inferior adrenal artery, which was elongated and displaced by the mass lesion.
