超音波監視下経皮的腎瘻術について

PERCUTANEOUS NEPHROSTOMY UNDER ULTRASONIC GUIDANCE

緒言

　上部尿路の閉塞性疾患に対する診断および治療として経皮的腎盂穿刺術は侵襲の少ない簡便な方法であるが，これを超音波監視下に行えば他臓器穿刺の危険なく，軽度の水腎症にも確実に施行できると報告されている^1,2）。また本法を用いて造設した腎瘻は上部尿路の閉塞のみでなく，下部尿路に機能的障害がある際の永久的な尿路変更としても利用し得るし，さらに一時的に造設した腎瘻を閉塞腎の重篤な感染症に対するドレナージや，機能回復の評価に用いることもできる。われわれは1982年1月1日より12月31日までの1年間に17例の経皮的腎瘻造設術を経験したのでその概要を報告する。

We practiced 17 percutaneous nephrostomies under ultrasonic guidance during last one year. Ourindication of this procedure were pointed out as follows;

1) As the simple method in place of conventional surgical nephrostomy.

2) To evaluate the restoration of function on the obstructed kidney.

3) For the management of acute pyonephrosis.
