前立腺肥大症術後の尿路管理

A STUDY ON THE POSTOPERATIVE URINARY TRACT MANAGEMENT FOR BENIGN PROSTATIC HYPERTROPHY

緒言

　前立腺肥大症術後の尿路感染症は最も普遍的な，複雑性尿路感染症の一型である。その対策として，1）術後のカテーテル管理の問題，2）カテーテル抜去後の管理，以上の2点が考えられる。われわれは1）としては点滴持続膀胱併用閉鎖式導尿法，2）に対してはカテーテル抜去直後より膿尿消失まで，定期的に膀胱洗浄を施行している。今回以上の2法につき，その効果を検討するため，本法施行前の本症症例との比較を行つたので報告する。

The occurence of catheter induced urinary infection following operation for benign prostatic hyper-trophy was studied in 139 cases, in which 23 cases were maintained by "open drainage system" (group 1), 24 cases by "closed drainage system" (group 2), and 92 cases by "closed drainage system with continuous bladder irrigation by polymixin B" (group 3). The infection rate at the time of catheter removal was 91.3% in the group 1, 25% in the group 2, and 13.0% in the group 3.
