下顎転移の先行した腎癌の1例

A CASE OF RENAL CELL CARCINOMA PRECEDED BY METASTASIS TO THE JAW

緒言

　腎細胞癌の初発症状は，ごく初期の内科的不定愁訴から尿路症状など多種多様にわたつている。さらに転移巣症状が先行する症例の存在も従来より注目されている。また従来からの報告では，口腔外科，耳鼻咽喉科領域への転移はかなり稀とされている。今回著者らは下顎骨への転移巣症状が先行し，その病理学的検索により原発巣としての腎細胞癌が発見された例を経験したので考察を加え報告する。

A 49-year-old man visited the department of oral surgery with complaints of analgesia of the lower lip and asymptomatic swelling of the left oral vestibule. Diagnosed as mandibular tumor, he underwent mandibulectomy. Pathological diagnosis was metastatic anaplastic carcinoma, so the patient was sent to urological clinic for detection of primary focus. Excretory urogram and CT revealed space occupying lesion in the lower pole of the left kidney. Transperitoneal nephrectomy was performed and patholo-gical examination revealed renal clear cell carcinoma, probable primary focus of the mandibular tumor.
