上部尿路疾患に対する背面からの到達方法の再検討

REAPPRAISAL OF LOMBODORSAL APPROACH FOR RENAL AND UPPER URETERAL SURGERY

はじめに

　従来より上部尿路に対する手術到達方法は腰部斜切開が広く用いられてきた。しかし，一方ではGil-Vernet法^1）に代表される背面皮切からの到達方法も優れた到達方法として評価を受けてきた。都立府中病院泌尿器科では上部尿路疾患に対して背面到達方法を第一次選択として手術を行つてきた。その結果を報告するとともに背面到達方法の手術的限界に対しても考察を加える。

The clinical experience with 115 operations using lombodorsal approach was analyzed. The operative procedure included 46 ureterolithotomies, 33 pyelolithotomies, 15 nephrostomies, 8 nephropexies, 8 open renal biopsies, 4 pyeloplasties and a case of bilateral ureterocutaneostomy.

The advantages of this approach were early ambulation and short hospitalization with minimal postoperative pain and minimal scarring. With this approach the renal pelvis was easily exposed, although the access to the vascular pedicle was difficult.
