はじめに
前立腺肥大症に対する手術的治療法としては前立腺摘除術があるが，この術式には恥骨上式，恥骨後式，会陰式，あるいは仙骨式などの方法があり，また他に経尿道的切除術（TUR-P）や前立腺凍結術などの方法がある。このうちいずれか一つの術式に熟達していれば前立腺肥大症に対する手術的療法は十分になし得るという考え方はもちろんできるが，各術式にはそれぞれの特長があり，症例によつてその術式を選択することも可能なはずである。われわれは都立府中病院での手術経験を元に各術式にみられる特長を検討してみたのでここに報告する。
Based on our experience of 401 cases of prostatic surgery for benign prostatic hypertrophy (185 retro-public, 154 perineal, 14 suprapubic, 6 sacral and 42 transurethral), various methods of prostatectomy were clinically evaluated.
In cases of retropublic prostatectomy, hemostasis and bladder neck reconstruction could be easily accomplished. Urethral catheter could be removed between first and third postopeative day.
This method was preferred for patients younger than about 75 years of age and without associated systemic diseases.
