泌尿器科領域における菌血症について

UROLOGICAL BACTEREMIA

緒言

　菌血症は適切な診断，治療で予後が大きく左右されるため，各科の臨床家にとつて，無視できない疾患である。その臨床経過は大体において各科共通なところが多いと考えられるが，基礎疾患，起因菌などで各科独特の特徴を有するところもあろう。われわれ泌尿器科医にとつても，本症は日常その対策に苦慮させられる疾患の一つである。よつて泌尿器科領域における本症の特徴を調べ，その対策の一助とするため，今回，過去5年間に当科で発生した菌血症症例の検討を行なつたので報告する。なお，菌血症の定義として今回は，悪感，発熱あるいは著明な白血球増多などの症状とともに，血液培養が陽性であつた症例はすべて本症として取り扱つた。

In order to investigate the clinical course and outcome of bacteremia in urological patients, data were collected retrospectively from all patients admitted to our department, in whom positive blood cultures were drawn. During 5 year periods 27 patients were registered with urosepsis corresponding to an incidence of 1.7%.

1) The cause of bacteremia was iatrogenic in 2/3 of the patients, especially in patients who were undertaken total cystectomy with ileal conduit.

2) Except for two cases of mycosis, the growth of gram-negative bacillus were observed in all cases, including 8 cases of Serratia marcescens infection.
