A 43 year-old man was admitted with complaints of fever and lower abdominal pain. Plain X-ray film showed the straight needle 4.0 cm in length with calcification near the left bladder wall. DIP revealed no excretion from the left kidney and ureteral catheter was unable to pass through the left ureteral orifice. Left ureterolithotomy was performed, which disclosed a sawing needle as the core of the ureteral calculus 5×1 cm in size.

It was impossible to define the route of the sawing needle to the left ureter. This is the fourth case of sawing needle as foreign body in the upper urinary tract in Japan.
