膀胱腫瘍における組織像の解析

ANALYSIS OF TISSUE IN IMAGES IN BLADDER TUMOR

緒言

　膀胱癌において，腫瘍の病理組織学的所見すなわち腫瘍の組織型，浸潤度（stage）そして悪性度（grade）と予後との間には密接な関係のあることが指摘されており，これらの所見にもとづいて治療法の選択や予後の推測がなされている。腫瘍の病理組織学的所見の一つである悪性度は，1922年Brodersが膀胱癌の悪性度は腫瘍細胞の正常膀胱上皮細胞からの違いの程度に関係することを指摘し，腫瘍細胞の分化度により4群に分類して以来腫瘍細胞の分化度による分類法として臨床的に用いられてきている。しかし，悪性度の判定には主観，部位，時期により違いがあり，必ずしも正確な判定を下すことはできないとされており，また判定方法もBrodersの原法以外に種々の分類法が発表されており統一されていない。したがつて悪性度が予後判定の因子としては不十分な点があり，その信頼性にも限度があるかと思われる。今回著者らは膀胱腫瘍の悪性度判定について再検討し，若干の知見を得たのでその信頼性について論じてみたい。

Tumor tissue images of totally extracted bladder samples in 50 cases of bladder tumor were analyzed and investigated with particular respect to difference of grade in various positions of tumor according to degree of cell differentiation and cell structure, its frequency, coexistence of various grades and tumor size, and relation between stage and prognosis. As a result, coexistence of varying cell types was noted in 8 out of 50 cases (16%), and coexistence of varying grades was seen in 21 cases (42%). Coexistence regarding both cell type and grade was observed in 3 cases (6%).
