先天性後部尿道弁の1例

POSTERIOR URETHRAL VALVE:A CASE REPORT

緒言

　先天性下部尿路通過障害のなかで後部尿道弁は最も重篤かつ高頻度に上部尿路合併症をひきおこすことで知られている。欧米においては多数例の報告がなされているが，本邦においては1977年の川村，寺島^1）のアンケート調査でも41例にすぎず，比較的まれな疾患と考えられる。最近われわれは本症の1例を経験したので若干の考察を加え報告する。

A six-year-old boy was admitted for the evaluation of a recurrent urinary tract infection. He complained of refractory bed-wetting but had no difficulty of urination.

Urinalysis showed mild pyuria. On a cystometric examination bladder capacity was 120 ml and maximal voiding pressure was 120 cm H₂O. On plain films spina bifida occulta and lumbosacral vertebra were seen. IVP was normal. Voiding cystourethrography showed bilateral vesicoureteral reflux with paraureteral diverticula and a sharp narrowing portion at the middle portion of the posterior urethra with a marked proximal dilatation.
