腎結石および腎周囲膿瘍を合併したXanthogranulomatous pyelonephritisの1例

A CASE OF XANTHOGRANULOMATOUS PYELONEPHRITIS WITH RENAL STONE AND PERIRENAL ABSCESS

緒言

　Xanthogranulomatous pyelonephritisは慢性腎盂腎炎のなかで，脂肪顆粒を有する泡沫細胞の増殖が著明な疾患である。欧米では100例をこえる報告があるが，本邦では18例の報告があるにすぎず，比較的耳新らしい疾患である。著者らは最近，腎結石および腎周囲膿瘍を合併した本症の1例を経験したので報告し，若干の考察を行なう。

Case is a 27 year old female, who was admitted to our clinic with the chief complaint of pain and swelling of right flank.

Plain film of the abdomen showed a stone at the right renal area, which coincided with the nonvisualizing upper calyx of right kidney by I.V.P.. Right flank incision was performed for drainage, on the day of admission, and B. proteus was cultivated. Nephrectomy was done on the 38 th day for continuous pain and swelling. Adhesion was remakable at the posterior wall of the kidney.
