結石腎にみられたXanthogranulomatous pyelonephritisの1例

XANTHOGRANULOMATOUS PYELONEPHRITIS WITH NEPHROLITHIASIS

はじめに

　Xanthogranulomatous pyelonephritisは慢性腎盂腎炎のうちで，脂肪顆粒を有する泡沫細胞（FoamcellまたはSchaumzellen）が著明に増殖し，肉眼的に黄色調を呈する特殊な型である。欧米ではすでに100例を越す本症の報告があるが，本邦においてはこれまで12例の報告があるに過ぎず，比較的最近になつて注目されるようになつた疾患である。著者は最近結石腎にみられた本症の1例を経験したので報告し，若干の考察を行なうこととする。

Case is a 34 year old female mill hand, with the chief complaint of fever and right flank pain, who visited and admitted to the urological department of Niigata University Hospital on June 2, 1970. There was tenderness at right renal area, plain film of the abdomen showed right renal calculi and I.V.P. revealed non-visualizing right kidney. Right nephrectomy was performed for suspicious right calculous pyonephrosis on June, 12. Gross appearance of removed kidney showed yellow spot-like tissue in the renal parenchyma near the pelvis with calculi.
