先天性仙椎欠損症と泌尿器合併症

AGENESIS OF THE SACRUM AND UROLOGICAL COMPLICATIONS

緒言

　仙椎の全欠損，正確には仙尾椎の全欠損またはその部分的欠損は，比較的稀有な先天性奇型であり，1852年Hohl^4）による死産児の症例が文献上の第1例である。欧米ではHohlの報告後1950年までにFreedman^3）により自己の1例を加えた45例が集められている。一方，わが国では，1929年金井・小室^5）による1例が報告され，その後1959年小川^7）による報告などを散見するにすぎない。しかし，しばしば重篤な泌尿器科的問題を合併しているにもかかわらず，泌尿器科的考察を加えた報告は少ない。わずかに1968年Warren^10）が，自己の8例を泌尿器科医の立場で報告している。最近，われわれは先天性仙椎欠損症の3例を経験したので，症例の報告と併せて，本症の泌尿器合併症について，若干の文献的考察を加える。

Three cases of sacral agenesis were presented. Partial or total absence of the sacrum is not so rare, but frequently serious urological complications are overlooked. Sacral agenesis is associated with various anomalies including urological disorders such as neurogenic bladder dysfunction, fused kidney, hydrone-phrosis, imperforated anus, hypospadia, rudimentary gonad and deformities of lower limbs. All of our cases had neurogenic bladder dysfunction with unilateral vesicoureteral reflux, suffering from urinary incontinence and infection. In the second case, aplasia of right kidney was revealed by I.V.P. and cystoscopic examination.
