先天性精管欠損症

CONGENITAL ABSENCE OF VAS DEFERNES

Ⅰ．緒言

　発生学的に男子においては，中腎導管（Wolff氏管）の上端は睾丸細精管と結合して副睾丸輸出管を形成し，中腎導管の中間部は精管を形成し，その下端部より精嚢腺，尿管，膀胱三角部が形成される。従つて中腎導管の発達過程の異常により，精路系と尿路系の先天異常が度々重複して起こり種々の所見を呈する。

　我々の教室では，先天性性器異常として1側Wolff氏体の完全欠損症（単腎単睾丸症）の1例を中島，藤枝（1961）が発表しているが，最近3例の先天性精管欠損症を経験した。これを機会に本邦文献上先天性精管欠損症として発表されている66例と，睾丸欠損症として発表されているが精管欠損症を合併していた4例を集め自験を加えた73例の先天性精管欠損症につき若干の文献的考察を試みた。

A case of congenital absence of left vas deferens in a 38 years old male, with the chief complaint of hemospermia, which was discovered during routine examination and 2 cases of congenital absence of bilateral was deferens with the chief complaint of male sterility, were reported and 73 cases of such congenital anomaly were collected from the Japanese literature and discussed.

Congenital absence of the vas were found in 1%, of male sterility and approximately 5% of azoospermia. 60% of them were complicated with other urological anomalies and 45% were anomalies of the epididymis.
