両側腎血管性高血圧症の1例

A CASE OF BILATERAL RENOVASCULAR HYPERTENSION

Ⅰ．緒言

　近年，腎性高血圧症の診断法の進歩とともに今まで原因不明のため，本態性高血圧症として治療されていたものの中に外科的治療により根治し得る腎血管性高血圧症が含まれ，その臨床例が多数報告されるようになつた。

　最近，我々も異型大動脈縮窄症，左腎内腎動静脈瘻を伴つた両側腎動脈狭窄による腎血管性高血圧症の1例を経験したので，この症例について述べ，あわせて若干の文献的考察を加えることとする．

Case is a 22 years old female who visited our clinic with the chief complaint of general fatigue and hypertension. Blood pressure was 184-180/82mm/Hg at upper extremity and 190-180/100mm/Hg at lower extremity which did not show a remarkable difference between them. Vascular murmur was audible from epigastrium to the umbilicus on the midline by auscultation. Remarkable hypofunction was noticed by both urea and renal clearance tests. Intravenous pyelogram showed some narrowing of the outline of the kidney, the pelvis and calyx of the right together with some difference in the contrast of the pelvis and calyx, bilaterally.
