Case is a 22 year old female who underwent partial nephrectomy of the upper 1/3 of the right kidney for right renovascular hypertension at the age of 18. Stricture of renal artery was noticed but there were 2 more abnormal blood vessels supplying the lower pole. Postoperative blood pressure showed no change.

She was admitted for hypertension 4 years later with the blood pressure of 220/180 to 150/110mm/ Hg and BUN was 9 mg/dl. PRA of separated renal vein were 3. 78 ng/ml/h on the right, 1. 71 on the left, 3. 0 below the junction of renal vein and vena cava and 2. 55 above the junction.
