緒言

　近年の腎性高血圧症に関する報告は多く，このことについてすでに検討しつくされたかの観がある。しかし，諸家の治療成績をみると良好な結果ばかりとはいえず，なお問題が残されているようである。最近われわれは腎部分切除術後も血圧下降のみられなかつた腎血管性高血圧症に対し，腎摘除術を行なうことにより高血圧を正常化できた症例を経験したので報告し，さらに腎血管性高血圧症に対する腎部分切除術について若干の考察を加える。

Case is a 22 year old female who underwent partial nephrectomy of the upper 1/3 of the right kidney for right renovascular hypertension at the age of 18. Stricture of renal artery was noticed but there were 2 more abnormal blood vessels supplying the lower pole. Postoperative blood pressure showed no change.

She was admitted for hypertension 4 years later with the blood pressure of 220/180 to 150/110mm/ Hg and BUN was 9 mg/dl. PRA of separated renal vein were 3. 78 ng/ml/h on the right, 1. 71 on the left, 3. 0 below the junction of renal vein and vena cava and 2. 55 above the junction.
