　症例は67歳の男性。2008年12月に左側腹部痛がみられ，近医を受診し，後腹膜腫瘍/脂肪肉腫の疑いで当科を紹介受診した。後腹膜腫瘍摘除・左腎合併切除術を施行した。病理組織診断で脱分化型脂肪肉腫と診断され，同時に切除した腸腰筋に浸潤が認められた。補助療法は行わず，現在外来で経過観察中である。脱分化型後腹膜脂肪肉腫について考察する。

　A 67-year-old man presented with left upper abdominal dull pain. An ultrasonography and a CT scans showed a large solid mass 10 cm in diameter with necrosis and bleeding and the fatty element surrounding the left kidney. Under a diagnosis of retroperitoneal liposarcoma,the mass and the left kidney were removed completely. Pathological examination of the tumor revealed dedifferentiated liposarcoma.
