初回尿細胞診class Ⅲ患者の診断に関する検討

Value of urine cytology class Ⅲ at first examination

　尿細胞診class Ⅲという結果については，その解釈に苦慮することが多い。初回検査時に尿細胞診class Ⅲの結果が得られた53例についてretrospectiveに検討した。最終診断については53例中29例（54.7％）が尿路上皮悪性腫瘍であり，そのうち膀胱癌を合併しない腎盂・尿管腫瘍の診断であったものが11例（20.8％）と比較的高率であった。悪性腫瘍以外の診断であった17例をみると尿路結石が9例と比較的多く，刺激型移行上皮によるものの影響が多いと考えられた。初回検査時に尿細胞診class Ⅲで，膀胱鏡で腫瘍を発見できなかった場合，上部尿路の精査が重要であると思われた。

　We are often confused with the result of urinary cytology class Ⅲ. We present a retrospective analysis of 53 patients with voided urinary cytology class Ⅲ at the first examination. Twenty nine（54.7％）of the patients sevealed urothelial cancer：eleven patients（20.8％）had upper urinary tract tumors without bladder tumor, showing the relatively high incidence of upper urinary tract tumors without bladder tumor. The findings indicate the important and of upper urinary tract examination when first voided urinary cytology results class Ⅲ. The contribution of the cytology of bladder washing specimens in the diagnosis of urothelial cancer was also emphasized.（Rinsho Hinyokika 57：435-438, 2003）
