膀胱子宮瘻の1例

A case of vesicouterine fistula

　症例は28歳，女性。帝王切開術後の腟性尿失禁を主訴に来院した。逆行性膀胱造影にて膀胱から背側への造影剤の流出を認めた。膀胱子宮瘻の診断下で，単純子宮全摘術および瘻孔閉鎖術を施行し，術後の経過は良好であった。膀胱子宮瘻は主として帝王切開術後に発症し，外科的治療により良好な予後を期待できる疾患である。

　A 28-year-old woman was referred to our hospital with a chief complaint of vaginal urinary incontinence after cesarean section. Retrograde cystography revealed a leakage from posterior bladder to uterus. Under the diagnosis of vesicouterine fistula, we performed closure of fistula after simple hysterectomy. Most of the vesicouterine fistula were thought to be generated by cesarean section, and expected to be cured by surgical treatment.（Rinsho Hinyokika 57：439-441, 2003）
