閉鎖神経に発生した後腹膜神経鞘腫

Retroperitoneal schwanoma developed in the obturator nerve：A case report

　患者は48歳，男性。健診で膀胱の左後部に超鶏卵大の囊胞状腫瘤を指摘され受診した。エコー，CT，MRIにて膀胱左後部に，膀胱を著明に圧排する囊胞性腫瘍を認めた。血管造影にて左閉鎖動脈を栄養血管とする腫瘍と診断した。以上より腫瘍摘除術を施行し，腫瘍は閉鎖神経より発生しており，病理組織はアントニAB混合型の神経鞘腫であった。術後7か月の現在，神経症状その他の合併症や再発を認めず経過観察中である。

A 48-year-old male was pointed out a large hen-egg-sized cystic mass at the left retro-vesical space. Ultra sound（US）,computed tomography（CT）,and magnetic resonance imaging（MRI）revealed that the mass was cystic tumor,and was fed from left obturator artery by angiography. Then we performed resection of the tumor,which was connected with the left obturator nerve. Pathologic diagnosis was schwanoma of AB mixed type by Antoni's classification. 7months after operation,the patient is free from neurological symptom without any evidence of tumor recurrence.（Rinsho Hinyokika 60：505-507,2006）
