S状結腸癌術後の精索転移

Metastatic spermatic cord tumor from sigmoid colon cancer：A case report

われわれは，S状結腸癌術後に右精索に転移性腫瘍を認め，切除した1例を経験したので報告する。症例は75歳，男性。70歳時にS状結腸癌にてS状結腸切除。術後経過観察中，2004年7月CEA上昇とともに右陰囊内容物の腫大を自覚した。右転移性精索腫瘍を疑い，同年8月右高位精巣摘除術を施行した。病理組織学的所見では中分化型腺癌で，S状結腸癌の転移と確診した。本例は転移性精索腫瘍の本邦報告90例目であり，リンパ行性転移であると考えられた。

Spermatic cord metastasis is relatively rare only 90 cases being reported in Japan. A 75-year-old man with a history of sigmoidectomy for sigmoid colon cancer at the age of 70,noticed a swelling mass in the right scrotum,and his serum CEA value was found to be elevated to 5.3ng/ml（normal：＜4.0ng/ml）. Under a preoperative diagnosis of metastatic sigmoid colon cancer to the spermatic cord,we performed a right high orchitectomy. Histopathological diagnosis was moderately differentiated adenocarcinoma,metastasis from sigmoid colon cancer. The postoperative course was uneventful without noticeable complications.
