咽頭痛を主訴とした咽後および縦隔気腫の1症例

A Case of Pharyngeal and Mediastinal Emphysema with Sore Throat

　はじめに

　皮下気腫および気胸は，気管切開術などの頸部手術の合併症としてよく知られている。しかし，手術および外傷の既往もなく皮下気腫，咽後気腫および縦隔気腫を呈する，いわゆる特発性縦隔気腫に耳鼻咽喉科医が遭遇することは稀である。咽頭痛は耳鼻咽喉科の日常診療で最も多い主訴の1つであるが，ときとして下咽頭腫瘍のような重篤な経過をたどる疾患の初期症状であることもあり，重要な症状である。

　われわれは，耳鼻咽喉科領域での視診検査では明らかな所見がないにもかかわらず，咽頭痛を強く訴えた咽後および，縦隔気腫の1例を経験したので，若干の文献的考察を加えて報告する。

A 18-year-old girl with idiopathic pharyngeal and mediastinal emphysema complained of sore throat. Thecervical X ray films showed free air in the retropharyn-geal space. CT revealed retropharyngeal and mediasti-mal emphysema without pulmonary bulla. As she had a previous history of severe cough, the cause of emphy-sema was considered as strong pressure in the pulmo-nary air way, which induced sudden occurrence of pha-ryngeal and mediastinal emphysema. She recovered without complication by bed rest within 8 days.
