眼窩内膿瘍をきたした前頭洞炎の1症例

A Case of Orbital Abscess following Frontal Sinusitis

　はじめに

　画像診断や抗生剤，抗菌剤が発達した現在では，副鼻腔炎による眼窩内合併症は著しく減少している。さらに，眼窩内合併症をきたしてもそのほとんどが適切な抗生剤投与で治癒し得るようになった。しかし，高度の視力障害を生じた症例や頭蓋内にまで炎症が波及した症例に対しては，早期の外科的治療が必要と考えられている^1）。

　今回われわれは，前頭洞炎から眼窩内膿瘍をきたし，外科的治療により眼窩上壁の骨穿孔を確認した症例を経験したので報告する。

A 70-year-old female was suffered from orbitalabscess following frontal sinusitis. She had pain of the right orbit, a swelling of the right eye lid, and the right exophthalmos. In spite of antibiotic treatment, she had impairment of visual acuity. External drainage of the frontal sinus was performed. It was revealed that there were some perforations on the floor of frontal sinus. We suspected that some perforations were caused by frontal sinusitis. After drainage her visual acuity improved.
