骨腫を伴う前頭洞炎から眼窩および頭蓋内合併症をきたした1症例

A case of orbital and intracranial complications from frontal sinusitis with osteoma

はじめに

　急性副鼻腔炎や慢性副鼻腔炎の急性増悪から眼窩内，頭蓋内へ炎症が波及し，種々の重篤な合併症をきたすことが知られている。今回われわれは，骨腫を伴う前頭洞炎の急性増悪により眼窩内合併症をきたし，その後に硬膜外膿瘍に至った1例を経験したので報告する。

　It is known that acute exacerbation of sinusitis can lead to inflammation in the orbit and intracranial space, resulting in a variety of serious complications. We describe a case of acute exacerbation of frontal sinusitis with osteoma resulting in intraorbital complications and subsequent epidural abscess. A 28-year-old man with no comorbidities presented with anterior forehead pain and bilateral eyelid edema. He was diagnosed with sinusitis with intraorbital complications and was treated conservatively without success, resulting in an epidural abscess. Subsequent surgical drainage improved the situation. Surgical intervention should be considered early in the course of a complicated sinusitis.
