 It is known that acute exacerbation of sinusitis can lead to inflammation in the orbit and intracranial space, resulting in a variety of serious complications. We describe a case of acute exacerbation of frontal sinusitis with osteoma resulting in intraorbital complications and subsequent epidural abscess. A 28-year-old man with no comorbidities presented with anterior forehead pain and bilateral eyelid edema. He was diagnosed with sinusitis with intraorbital complications and was treated conservatively without success, resulting in an epidural abscess. Subsequent surgical drainage improved the situation. Surgical intervention should be considered early in the course of a complicated sinusitis.
