頭皮に原発した血管肉腫の1例

A Case of Angiosarcoma of the Scalp

　はじめに

　血管肉腫は，高年男性の頭部から顔面にかけて好発し，皮膚科もしくは耳鼻咽喉科を受診する機会の多い間葉系悪性腫瘍である。本邦ではこれまでに100例余りの報告がある^1）。悪性度が極めて高く，これまで効果的な治療法はほとんど確立されていない。しかし，近年IL-2の局所または全身投与と電子線照射の併用が有効であるとの報告が散見される^2,3）。

　今回われわれは，頭頂部皮膚に現発し頸部転移を生じた血管肉腫を経験したので報告する。

A 77-year-old male with a 30 X 30 mm angiosar-coma of the scalp was reported.

The patient noticed a painless nodule in the scalp. The nodule was incised and drained under diagnosisof skin abscess, but the nodule became ulcerative in several weeks. Two months after the initial treatment, biopsy revealed malignant hemangio-endothelioma. The surgical treatment and irradia-tion were not indicated because of multiple lung metastasis. Intravenous injections of IL-2 was ad-ministered for 15 days without remarkable response. The patient died of local reccurrence, cervical lymph node metastasis and distant metastasis of the lung.
