多発性対称性脂肪腫症の1症例

Multiple Symmetric Lipomatosis:A Case Report

　はじめに

　多発性対称性脂肪腫症（multiple symmetric lipomatosis：MSL）は1888年，Madelung^1）によって初めて報告された比較的少ない疾患である。巨大な脂肪腫が頸部，背部，体幹などにび漫性，対称性に生じ，独特な外観を呈するため疾患に対する認識があればその診断は容易である。治療には難渋する場合が多いが，今までの報告では手術内容の詳細については述べられていない^1〜5）。

　今回われわれは，MSLの典型例と考えられる1症例の治療経過を報告し，この疾患に対する手術療法について考察した。

A 61-year-old man with multiple symmetric lipomatosis (MSL) presented with multiple masses in the cervical, preauricular, postauricular, andsupraclavicar regions of the neck. This patient had been habitual alcoholic for over 40 years. Labora-tory investigations revealed liver dysfunction and hyperuricemia. We performed surgical resection without liposuction and endoscopic surgery. Surgery in several stages was essential because of the extent of multiple masses and adhesion. There was no sign of recurrence postoperatively for over a year.
