　はじめに

　反回神経麻痺の原因疾患は多岐にわたり，これまでも多くの症例，臨床統計が諸家により報告されている。中でも胸腔内疾患，特に胸部大動脈瘤を原因とする反回神経麻痺は，一般的にはよく知られているのに反し，その発症頻度は低く，またその多くは他科で既に診断が下されていることが多い。

　今回われわれは，嗄声のみを主訴とし，最初に耳鼻咽喉科を訪れた解離性大動脈瘤の2症例を経験したので，症例を報告するとともに，動脈瘤の反回神経麻痺に占める頻度や回復症例について考察した。

A 75-year-old male and a 78-year-old female presented to our hospital with hoarseness. The leftvocal cord was fixed in the paramedian position in each patient. In the first case, chest x-ray revealed right deviation of trachea, and the second case revealed a large mass occupying the left upper lobe of the lung. These two cases were diagnosed as having dissecting aortic aneurysm by CT-scan. It is emphasized that dissecting aortic aneurysm shows only hoarseness as an initial symptom.
