上咽頭癌局所再発例に対する動注化学療法

A Case of Recurrent Nasopharyngeal Squamous Cell Cancer Responded to Intra-arterial Infusion Chemotherapy

　はじめに

　抗腫瘍剤の局所動脈内投与法は，1950年にKloppら^1）が試みて以来，静脈内投与に比べ優れた抗腫瘍効果と副作用の軽減を図る方法として臨床的にも広く行われてきた。頭頸部領域においても，とりわけ上顎癌に対する浅側頭動脈を利用した持続動注療法＋放射線治療は，集学的治療の一環として確立された治療法であり，その他の悪性腫瘍に対してもセルディンガー法にて超選択的動注化学療法を実施している報告^2〜4）もある。

　今回われわれは，抗腫瘍剤の全身投与では効果が認められなかった頭蓋内浸潤をきたした上咽頭癌局所再発例に対し，超選択的動注化学療法および浅側頭動脈からの持続動注化学療法を行い，有効であったので文献的考察を加えて報告する。

A 59-year-old man treated with radiotherapy of the nasopharynx and neck lymph nodes for nasopha-ryngeal cancer 5 years earlier, presented with acomplaint of diplopia. Fiberoptic examination of the nose and nasopharynx revealed ulcerative les-ion. Transnasal biopsy revealed squamous cell car-cinoma. Invasion of the ethmoid sinus, sphenoid sinus and right lobus temporalis was identified by MRI. He received 2 courses of systemic chemother-apy with MTX, 5-FU and CDDP, but no response of the tumor was obtained. As the second treatment, superselective and selective intra-arterial infusion chemotherapy was performed. After the treatment, majority of the tumor mass disappeared on MRI.
