長期間のバルサルバ法により発症した気脳症症例

A Case of Pneumocephalus Caused by Valsalva's Maneuver

　はじめに

　バルサルバ法は，18世紀初頭イタリア人のバルサルバにより提唱されて以来，現在自己通気として最も知られており，滲出性中耳炎の治療として奨励している施設がある。今回われわれは，滲出性中耳炎にて近医で耳管カテーテル通気を施行されていたが，その後バルサルバ法を長期続けた結果，眼窩，頭蓋内に気腫が発生して眼球突出，複視が出現した症例を経験した。カテーテル通気により粘膜下気腫を起こすことは時にあるが，バルサルバ法で頭蓋内まで至る気腫が発生した例は極めて稀と思われたので，若干の考察を加え報告する。

Head CT of a 68-year- old man with exophthal-mos and diplopia revealed pneumocephalus andorbital emphysema. It was considered that these were caused by Valsalva's maneuver the patient performed repeatedly for a long period of time, therefore the patient was instructed to discontinue this maneuver. Consequently, the pneumocephalus was absorbed and the orbital emphysema was im-proved. it was believed that the pneumocephalus was caused by a defect in the petrous bone.
