　はじめに

　血瘤腫は比較的まれな疾患で，ほとんどが上顎洞に発生し，鼻腔・篩骨洞に進展する。しかし本疾患名は臨床上の名称であって，病理組織学的には血管腫や壊死組織が多い。またその成因についても，多くの報告があるが定説はない。さらに臨床症状，検査所見からも悪性腫瘍との鑑別が問題となることが多い。今回われわれは悪性腫瘍を疑った上顎洞血瘤腫を経験したので，鑑別診断や成因についての若干の考察を加えて報告する。

A 22-year-old man complained of a swelling of the left eye lid, nasal obstruction and nasal bleeding. Radiographic examination showed a diffuse and irregular shadow with destruction of the posterior and superior wall of the left maxillary sinus. Tenta-tive diagnosis was malignant tumor of the maxil-lary sinus, but biopsy of the nasal cavity showed nasal mucosa with inflammation. The patient was treated with maxillary and ethmoidal sinectomy. Histopathological examination revealed no evi-dence of malignancy, but hematoma, necrotic tissue and fibrous tissue with a part of hemangioma-like lesion.
