当院における結核性頸部リンパ節炎の検討

Tuberculous Cervical Lymphadenitis:Report of Four Cases

　はじめに

　近年，結核の新登録患者数は年々減少しているものの，その減少速度は鈍化しているという^1）。

　耳鼻咽喉科領域においても，結核性頸部リンパ節炎は，以前に比べて遭遇する機会は減少したものの，決して稀な疾患ではなく，頸部リンパ節腫脹を認めた場合，常に鑑別として念頭に置いておかなければならない疾患である。しかし，結核に対するわれわれの認識が薄れてきているために，必要な検査を怠り病理組織生検を施行して初めて結核であることが判明することもしばしばあるのが現状である。

　今回われわれは，過去4年間に4例の結核性頸部リンパ節炎症例を経験したので，反省点，考察を加えて報告する。

During 1993-1996, we had four patients of tuber-culous cervical lymphadenitis. One of them was one year old. It is necessary to rule out tuberculous lymphadenitis, when we see patients with cervical lymph node swelling.

Open biopsy is useful for the diagnosis of tubercu-lous lymphadenitis. ESR, tuberculin skin test, spu-tum test and PCR assays for Mycobacterium tuber-culosis are also useful for the diagnosis.
